Your username will be preset using the following format, using all lowercase letters and no spaces: first two letters of first name, full last name, 4-digit birth year

Example: John Smith, born in 1970
Username: josmith1970

Click the link to https://www.revolutionphr.com and click Forgot Password. A password reset link will be emailed to you. Create your own unique password and return to the portal login page to enter your username and new password.
